At CHC/SEK we provide early detection, intervention, and preventive measures to support the growth and development of your child.   

At most 0 – 5 year Well Child visits, we ask parents to complete a developmental screening for their child. This helps us celebrate your child’s milestones and determine whether follow-up steps are needed. We ask you to complete the Developmental Screening tool (ASQ) one week before your appointment. Fill out the screening here.
