At CHC/SEK, we help children grow and thrive through every stage of development. Our team provides compassionate, personalized care for newborns, school-age children, and teenagers.
At most 0 – 5 year well-child visits, we ask parents to complete a developmental screening for their child. This helps us celebrate your child’s milestones and determine whether follow-up steps are needed. We ask you to complete the Developmental Screening tool (ASQ) one week before your appointment. Fill out the screening here.
