GENERAL DESCRIPTION

This position is part of the case management services team. This position provides services for any pregnant women, children, or families needing case management. This position completes initial interview to determine patient and family needs both health and social.  Provides support to utilize resources within the organization, as well as community resources that may be needed. On occasion home visits are necessary.

ESSENTIAL DUTIES

  1. Provides considerate respectful care focused upon patient and/or family individual needs.
  2. Accepts referrals of expectant mothers of all ages, children/families in need of care management (such as children with special health care needs, etc), and school-based health centers for children in need of support or follow up.
  3. Provide initial intake for prenatal patient (completing OB history, medical history, etc) and following care throughout pregnancy and after delivery.
  4. Facilitate appointments for well child, prenatal, immunizations, medical, dental, behavioral health, family planning, STI testing/treatment, etc. and follow up to make sure appointments are kept.
  5. Maintain contact with patients/families at least monthly until services are no longer needed.
  6. Works with patients to establish personal and professional goals and supports achievement of those goals.
  7. Assess patient and/or family needs both medical and social.
  8. Provides pre-natal and parenting education on best practices, including breastfeeding, safe sleep, proper car seat installation, etc.
  9. Coordinates expectant mothers and children’s medical visits with dental and mental health visits; works with the dental care coordinator.
  10. Works with MCOs to ensure children (especially adolescents) are accessing health care per the EPSDT schedule.
  11. Applies “ages & stages” developmental screens and provides these services in-clinic and during outreach, as appropriate.
  12. Have knowledge of CHC/SEK and community resources in order to advocate and provide assistance for patients. Connects patient and/or family with resources within CHCSEK and community.
  13. Maintains appropriate level of direct contact with families per program requirements, and remains available to families 24/7, for crisis intervention as needed.
  14. Works as a team member with other case managers/care coordinators to avoid duplication and/or gaps in service provision.
  15. Ensure patients with special needs, and their families, have access to healthcare (medical, dental and behavioral services if needed).
  16. Assists with Medicaid, Presumptive Eligibility and Insurance Marketplace enrollment.
  17. Organizes and coordinates at least one special activity (with collaborating partners) annually focusing on child health or safety such as a bicycle safety fair, car seat check, etc.
  18. Tracks referrals made to other non-CHC/SEK providers to ensure continuity of care and to meet reporting requirements.
  19. Coordinate an advocacy group, including regular meetings and communication with group keeping them current on appropriate issues.
  20. Participates in interagency coalitions throughout Southeast Kansas; promotes MCH services to participating agencies.
  21. Coordinates and/or provides patient transportation as needed.
  22. Meets all state and federal grant requirements, including reports and data entered into Daisey.
  23. Attends Kansas Governor’s Conference/ Public Health.
  24. Performs other duties as assigned.

QUALIFICATIONS

  1. Minimum of high school diploma.
  2. Experience in a healthcare setting and/or social work, preferred.
  3. BLS certification upon hire or obtained within six (6) months from date of hire.
  4. Able and willing to work on an as-needed basis in order to meet operational needs of the clinic.

  9. Maintain patient confidentiality in accordance to organization’s policy and procedure and HIPAA requirements.

WORKING CONDITIONS

While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is frequently required to sit.  The employee must occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision, peripheral vision and ability to adjust focus. The noise level in the work environment is usually moderate. Hazardous material inventory and MSDS available. Wears personal protective equipment as needed. May be exposed to patients/public with communicable diseases. May be required to conduct home visits.
