Payments, Insurance and Financial Assistance
Medical
CHC/SEK accepts Medicare, Medicaid, HealthWave and private insurance. Its providers are members of Pinnacle Care, the hospital/physician organization that contracts with different insurance companies on behalf of its members.
Dental
Medicaid, HealthWave and private dental insurance is accepted. Please contact your insurance carrier directly if you have any questions on your coverage. You are responsible for any non-covered services.
Mental Health
Medicare, Medicaid, HealthWave and private insurance is accepted. Please contact your insurance carrier directly if you have any questions on your coverage. You are responsible for any non-covered services.
No Insurance?
We will help you determine your eligibility for various programs offered by the state in which you reside. If you do not qualify, we offer discounted fees based upon family size and income level. Please bring a completed Financial Assistance Form (see versions below) and proof of income with you at the time of your visit. Approval for financial assistance is valid for one year. CHC/SEK does not refuse service to anyone based on ability to pay. Please call or visit one of our clinics for more information.
