Medical Appointments

Please call the medical clinic nearest you to schedule an appointment. For routine appointments, every effort will be made to schedule you within two to five days. Same-day urgent care appointments are available for children and, on a limited basis, for adults. Please call in advance to schedule a time. You should plan to arrive at least 15 minutes before your scheduled appointment.

Dental Appointments

Please call the dental clinic nearest you to schedule an appointment or 1-866-396-6788. Children and adults can receive urgent care (for pain, infection, etc.) on a priority basis and should call 1-866-396-6788 to determine which clinic has the first available appointment. We provide ongoing, comprehensive dental care to children ages 0-18 years old. Because of the high demand for our dental services, we are only able to offer on-going care to adults who qualify through the Adult Dental Program. An application is required and space is limited. Please call 1-866-396-6788 for information on qualifying for this program.

All new patients are required to fill-out the New Patient Information Form and bring it to their first appointment.

What to bring to your first appointment:

• Medical Insurance Card
• Proof of income (pay stubs, tax return) if you are applying for discounted services
• A list of all medications you are taking (or bring the medications with you).
